Apply to Student Ability Success Center

In order to determine eligibility for services, the following steps need to be taken:

Step 1:

All students must complete and submit an Application for Services (Word / PDF).

Step 2:

Students with learning disabilities

Students with AD/HD

Students with a psychological disability

Students who are deaf or hard-of-hearing

Students with visual limitations

Students with physical disabilities: mobility or health-related

Students with Acquired Brain Injury (ABI)

**Applications and documentation can be faxed, mailed, or hand delivered to the SASC office.  Documents can also be scanned and emailed to: sascinfo@sdsu.edu.  The review process can take up to a few weeks, depending on the number of applications that have been submitted.  You may be contacted if additional documentation is needed to determine eligibility.

Step 3

Once eligibility has been determined, you will be contacted to schedule a meeting with an assigned SASC Advisor for an Intake/Information appointment.  Your intake appointment will review needed accommodations and other services for which you qualify.
