To find out if you are eligible, go to HealtheConnect.sdsu.edu to make an appointment with Health Promotion for Family PACT certification, where we will determine if you are eligible.

Please download the Family PACT Client Eligibility Certification form (PDF), complete it, and bring it with you to your appointment.

 

 

 

Note: Your eligibility appointment is not a medical appointment! You will need to make a separate appointment with Student Health Services in order to get medical care.

About Family PACT

Family PACT is a State of California limited benefits program with a focus on family planning.  Family planning related reproductive health services are covered for cost, when the services are provided as part of, or a follow-up to, a family planning visit.

Women and men are eligible for the Family PACT Program if they reside in California, are at risk of pregnancy or causing pregnancy, have a gross family income at or below 200% of the Federal Poverty Guideline (FPG), and have no other source of health care coverage for family planning services. The Family PACT Program is administered by the California Department of Public Health, Office of Family Planning (OFP).

The overall goal of the Family PACT Program is to ensure that low-income women, men and teens have access to health information, counseling, and family planning services to reduce the likelihood of unintended pregnancy and to maintain optimal reproductive health.
