Facility Inspection and Program Review Guidance

Semiannual Institutional Animal Care and Use Committee (IACUC) inspections of animal facilities and animal use areas (laboratories) and semiannual IACUC review of institutional animal care and use programs are required by Animal Welfare Act (AWA) regulations and the Public Health Service Policy on Humane Care and Use of Laboratory Animals. These regulations, as well as SDSU's institutional policy, require compliance with two sets of standards: the AWA regulations and those in the Guide for the Care and Use of Laboratory Animals. The AWA regulations and the Guide contain many specific requirements applicable to physical plant, environmental controls, husbandry procedures, and institutional programs such as personnel training, veterinary care, and occupational health. One of the IACUC's tasks is to confirm that SDSU's facilities and programs comply with the regulations and institutional standards).

The semiannual program and facility checklist is a tool designed to assist the IACUC in conducting thorough semiannual reviews. This checklist covers the major topics of the Guide and the requirements of the PHS Policy. The program review section is titled Semiannual Facility Inspection Checklist. This program review section is further divided into two parts that correspond with Chapters One and Three of the Guide: Institutional Policies and Responsibilities and Veterinary Medical Care. This program review is completed during the semi-annual inspection.

The Semiannual Program Review Checklist corresponds to Chapters Two and Four of the Guide. It is further divided into animal housing and support areas, cagewash, aseptic surgery, and procedure areas, non-survival surgeries, laboratories, and rodent surgeries. Because these are physical aspects of the program that require visual observation to evaluate, this form is filled out during the walk-through inspection.

Any deficiencies noted during this semiannual inspection and program review will be reported to the Principal Investigator and/or the Office of Laboratory Animal Care Manager to correct. A written plan of correction must be included in the semiannual report to the Institutional Official.

Exceptions to the standards and regulations must be specified and explained by the Principal Investigator and approved by the IACUC. A summary of all such exceptions must be attached to the facility's annual report to the USDA.

Any failure to adhere to the correction plan and schedule that results in a significant deficiency remaining uncorrected must be reported in writing within 30 business days by the IACUC, through the Institutional Official, to the USDA and any Federal agency funding that activity.
