Request for Other Service Forms

Request for Completion of Out-of-State Form (transfer of license request)

Instructions:

  • Step 1. Complete the out of state form online and attach your request verification form.
  • Step 2. Electronically sign your request and click on “Sign Form” to send in your request
  • Step 3. To successfully send your request electronically to the Office of Student Services, you must validate your email address as the final step to confirm submission of your request. Please check your email immediately after you click on “ SIGN Form” and look for an email from Echosign@echosign.com. This email may be filtered to your junk or spam folder.

Request for Supplementary & Subject Matter Authorizations Form

Instructions:

  • Step 1. Complete the Supplementary & Subject Matter Authorizations form online and attach your transcripts as applicable. Attachment is not required if you have all transcripts on file with SDSU.
  • Step 2. Electronically sign your request and click on “Sign Form” to send in your request
  • Step 3. To successfully send your request electronically to the Office of Student Services, you must validate your email address as the final step to confirm submission of your request. Please check your email immediately after you click on “SIGN Form” and look for an email from Echosign@echosign.com. This email may be filtered to your junk or spam folder.

Request for CSET Waiver Letter Form

Instructions:

  • Step 1. Complete the CSET waiver letter request online and attach the single subject credential program advisor’s recommendation form signed by your single subject waiver program advisor.
  • Step 2. Electronically sign your request and click on “Sign Form” to send in your request.
  • Step 3. To successfully send your request electronically to the Office of Student Services, you must validate your email address as the final step to confirm submission of your request. Please check your email immediately after you click on “SIGN Form” and look for an email from Echosign@echosign.com. This email may be filtered to your junk or spam folder.

Request for Status (Enrollment Verification) Letter Form

Instructions:

  • Step 1. Complete the Status Letter form online
  • Step 2. Electronically sign your request and click on “Sign Form” to send in your request
  • Step 3. To successfully send your request electronically to the Office of Student Services, you must validate your email address as the final step to confirm submission of your request.
